UPDATE ON CORONAVIRUS (COVID-19) MAY 2022
PHILLIPS RESPIRONICS RECALL OF ALL CPAP/ BIPAP MACHINES: You should contact your homecare company for a replacement machine- we are unable to provide you with a replacement. If CPAP has been prescribed by your sleep doctor to control symptoms of sleep apnea including daytime sleepiness and low oxygen at night, the doctors are recommending to stay on CPAP until such time as a replacement machine is provided for you. We may have appointments available to speak to a doctor at short notice. You should email us at reception@sleep-clinic.ca if you require any further information. Our doctors' appointments are still mostly be telephone. You will be notified well in advance of an appointment if your visit is to be at the clinic in person with the doctor. There are no in-person visits for daytime appointments in either CMH or St Catharines. Please pay careful attention to the emails and sms sent to you.
You are required to fill in a PATIENT QUESTIONNAIRE- CLICK HERE before an appointment can be booked. Once we receive the questionnaire you will be advised of the date of your phone appointment or sleep study. Phone us if you cannot complete the online form- we can arrange for it to be mailed to you. Please make sure you fill in the correct form- the long form for a first appointment and the FOLLOW UP PATIENT QUESTIONNAIRE- CLICK HERE for any repeat appointment to check up on therapy.
